Does not comply……..”Whilst processes for identification and assessment of residents’ care needs, care planning, care delivery, referral and evaluation of the effectiveness of care are in place, treatment requirements and care directives are not consistently implemented. Follow-up of acute care issues and the effectiveness of treatment is not consistently monitored and documentation procedures are not consistently followed. Reassessment is not consistently undertaken when there is a change in residents’ response to care. This has resulted in negative clinical outcomes for residents.”
