Residents’ mobility and dexterity are optimised by planned exercise programs and strategies to reduce their assessed risk of falls. The home’s visiting physiotherapist assesses all residents. A mobility care plan and individual exercise program is documented and residents are assisted with exercises by care staff and the home’s allied health assistant.
Improvements in resident mobility have been noted following regular planned exercises. Residents have access to mobility and dexterity aids to meet their assessed need for support and to maintain their independence. Staff are alerted to residents at risk of falls by a ‘traffic light’ system in resident wardrobes. Additional strategies to reduce falls and injury may include ‘sit to stand’ poles attached to resident beds, sensor mats and hip protectors. The home monitors the incidence of falls and analyses potential causes. This process may trigger resident reassessment
or changes to levels of staff support. Residents are satisfied with the home’s approach to encouraging resident activity and optimising their mobility.
Marten Residential Care Centre (ECH) RACS ID: 6964C
The home has processes for assessing residents’ falls risk level, mobility and transfer needs and preferences when they enter the home, and planning effective strategies to minimise the frequency and impact of falls. Physiotherapy assessment including limb strength, range of movement and balance is provided for all residents, and programs are implemented to provide rehabilitation or maintain residents’ mobility levels. Nursing staff and a physiotherapist regularly review residents’ progress, and the frequency of review is increased according to changes in residents’ health status. Staff carry out exercise programs recommended by a physiotherapist and are aware of, and have access to the equipment required to safely transfer residents who are unable to weight bear. The home has processes for reporting and investigating resident falls to identify the causes and implement actions to minimise the future risk of falls. There are processes for monitoring staff practice relating to residents’ mobility and dexterity. Residents and relatives are satisfied with the way staff support them to mobilise where required and the equipment provided to assist them to safely maintain their independence.

A physiotherapist conducts mobility assessments, initiates exercise programs, falls risk assessments and helps residents to select and fit walking aids. Care workers, lifestyle staff and the physiotherapist assist residents needing individual exercise programs. Group exercise programs are also part of the activities program. The physiotherapist reviews effectiveness of the programs on an individual basis. Each resident who experiences a fall is reviewed by the physiotherapist. The home provides access to wheelchairs, motorised scooters, mobility and dexterity aids for residents who are assessed with an identified need. Attention to the physical environment supports safe and independent mobility. Specific strategies include manipulating the height of beds, adjusting the placement of furniture in resident rooms and using sensor mats to minimise the incidence of falls. Residents and representatives are satisfied with the assistance and range of aids provided by the home.

The home has access to a physiotherapist each week to oversee residents’ mobility, dexterity and rehabilitation programs, podiatry services are also available. Residents are assessed on entering the home to identify mobility and dexterity deficits and to identify any rehabilitation requirements. A regular review of care and progress occurs. An exercise program is in place and residents are assisted and encouraged to attend. The activities program demonstrated that specific activities address mobility and dexterity. The physiotherapy aide supervises mobility programs and receives education from the physiotherapist. The home conducts environmental audits regularly to ensure resident and staff safety. Incident and hazard forms are utilised and followed through to resolution and referred to occupational health and safety meetings for review. Residents and representatives confirmed that residents are encouraged and assisted to attend exercise classes and that the physiotherapy aide supervises programs implemented by the physiotherapist.
