Chronic Disease Community Program

Are you interested in managing your condition better? The Chronic Disease Community Program may be able to help.
A chronic disease is one that persists over a long period of time. Often it can affect your day to day life and activities. Things you could once do very easily may now be difficult or impossible to achieve.
However, there are steps you can take which may help slow the progression of your disease and help you manage your symptoms better.
The Southern Chronic Disease Community Program works together with your GP and community based health services to provide a range of self-management options that can help you manage your disease better by:
- Addressing specific risk factors to try and slow the progression of your disease, for example physical activity, smoking and nutrition.
- Introducing self -management strategies to help you manage your disease more effectively in your own community, for example home exercise programs, relaxation skills and managing medications better.
There are many ways the Southern Adelaide Health Service can help you manage your health better. These
include:
- Allied Health Services such as physiotherapy, occupational therapy, podiatry, nutrition and dietetics, social work or speech pathology. Services such as this can help you learn skills to manage your symptoms better and improve your quality of life once you leave hospital. Wellness & Lifestyles Australia is an accredited approved provider on the Southern Adelaide Health Service Chronic Disease Community Provider Panel.
- Chronic Disease Self-Management Programs. These programs – which range from one-on-one sessions to groups essions – can provide you with skills to better manage your condition. Carers and support people are welcome to come along with you.
- Home Medicines Reviews. A pharmacist will visit you in your own home to discuss what your medicines are for, the best way to take them and how to store them.
- Diabetes Group Education. This is a three-week course for people with Type 2 Diabetes.
- Telephone Coaching. Once you get home you can receive regular telephone aclls from a designated ‘coach’. The role of the coach is to support you to achieve your goals and improve the management of your disease, such as increasing your physical activity or giving up smoking.
- Oral Health Service. Oral health is important to our overall health. This service offers assessment, education and advice by a dentist regarding your oral health needs. Some treatment is also available.
What does it cost? Most of the services offered as part of the program have no cost to you. Dental services may involve a small co-payment.

Physical Activity Provider Panel
Wellness & Lifestyles Australia is an accredited approved provider on the Southern Adelaide Health Service Physical Activity Provider Panel.
W&L has undertaken to provide the following services:
W&L will provide structured physical activity programs and services for people who are over 18 and who have a chronic condition or are at risk of developing chronic conditions (e.g. cardiovascular disease, including heart failure and angina; diabetes and Chronic Obstructive Airways Diseases), and which assist in increasing their capacity to manage their chronic condition.
Wellness & Lifestyles Australia has specifically designed physical activity programs and services for:
- General physical activity programs
- Programs for older adults
- Falls prevention programs
- General chronic disease
- Well-compensated chronic heart disease
- Advanced chronic heart disease
- Older people with cardiovascular disease
- People with heart failure
- Type 2 diabetes
- Chronic Obstructive Pulmonary Disease
W&L’s physical activity programs and services reflect the following:
- Tailored to the needs of individual participants within the parameters of the program and with the capacity to be flexible in response to the changing needs of the participant.
- Provide a service that will be determined through a process of clinical (where appropriate) and functional assessment, negotiation and planning with participant.
- Provide services within a self-management and health promotion framework.
